I Need a Bone Graft Before My Dental Implant. What Does That Mean?

"I thought I was just getting a dental implant. Why do I also need a bone graft?"


The short answer is that a dental implant needs enough healthy bone around it to remain stable for many years. In some cases, there isn't enough bone to support the implant properly, so a bone graft may be recommended before, or at the same time as, implant placement.


It's quite common amongst patients and it doesn't mean your implant treatment can't go ahead as scheduled.



Why isn't there enough bone?


Your jawbone exists to support your teeth. Once a tooth is removed, the surrounding bone no longer receives the normal stimulation that comes from chewing. Over time, it is completely normal for that area of bone to reduce in height and width.


Some people lose very little bone. Others lose more. The amount depends on factors such as how long the tooth has been missing, whether there was infection before the extraction, gum health, smoking habits and individual healing.





So what does a bone graft actually do?


A bone graft increases the amount of bone available to support a dental implant.

It provides a scaffold that allows your own bone to grow into the area during healing. As that healing progresses, the graft becomes integrated with your natural bone, creating a stronger foundation for implant treatment.



How do we know if a bone graft is needed?


We don't decide this by looking in your mouth alone. Planning begins with a clinical examination and imaging. For many implant cases, a CBCT (Cone Beam Computed Tomography) scan is used because it produces high-resolution, three-dimensional digital images of teeth, jawbones, nerve pathways, and surrounding tissues. This allows us to measure the available bone accurately, assess its width and height, and identify important anatomical structures such as the maxillary sinus and the inferior alveolar nerve. Then can we decide whether there is enough bone to place an implant safely.



Does everyone having an implant need a bone graft?


No. Many patients have enough healthy bone and can proceed straight to implant placement.


Bone grafting is only recommended when additional bone is needed to improve the stability and long-term predictability of the implant.



Where does the graft come from?


Patients often ask whether we take bone from somewhere else in the body.

Sometimes your own bone is used, but today there are several well-established grafting materials available.


These include bone material that has been specially processed for medical use, bovine-derived bone substitutes and synthetic grafting materials.



Is it painful?


The procedure is carried out under local anaesthetic, so you shouldn't feel pain while the graft is being placed.


Afterwards, some swelling and tenderness are normal. Most patients describe the recovery as similar to having a tooth removed, although this varies depending on the size of the graft and the area being treated.



Will it delay my implant?


If there is enough bone to achieve good implant stability, the implant and graft can often be placed during the same appointment.


If more bone is needed, the graft may be allowed to heal first before the implant is placed.


Your treatment plan is based on what gives the implant the best chance of long-term success, not simply what can be done most quickly.



Does needing a bone graft mean I'm not suitable for implants?


For many patients, bone grafting is what makes implant treatment possible.

Its purpose is to create enough healthy bone to support an implant properly, giving it the best chance of functioning successfully for many years.



It sounds scary, but it is a common procedure.


Being told you need a bone graft can sound like an unexpected setback. In reality, it is a routine part of implant dentistry for many patients.


The decision isn't based on guesswork. It's based on careful assessment of the quantity and quality of the available bone, together with planning that is designed to give your implant the strongest possible foundation.
